Want to help influence recommendations for health-related service improvements in Roseville? The Roseville Block Nurse Program (BNP) Reinvention Task Force is asking you to help them to learn about the health care needs of the community.
Please complete a health needs survey and encourage your family, friends and neighbors in Roseville to do the same. The anonymous survey takes about 20 to 30 minutes to complete. With more diverse responses, the better recommendations can be.
- Roseville Senior Health Needs Survey is available at www.surveymonkey.com/s/7W3NJ5P. Roseville Senior Health Needs Survey is also available in hard copy at Roseville City Hall. Anyone 60 years or older is encouraged to take the Senior Health Needs Survey.
- Roseville Adult Health Needs Survey 2 is available at www.surveymonkey.com/s/YK5XHXR. Anyone under 60 years of age is encouraged to take the Adult Health Needs Survey.
The surveys are part of the efforts of the Roseville Block Nurse Program (BNP) Reinvention Task Force, a volunteer effort of citizens interested in public health authorized by Roseville City Council in October 2014. The task force will present its results to the City Council later this summer.
